Case History: Kirti was studying in 11th grade and was also preparing for her medical entrance exam. She was diagnosed with PCOS and was also suffering from high insulin resistance of around 30. Her BMI levels suggested that she was obese with 88kg weight. Her health issues hampered with her study and exams schedules. She had irregular periods causing mood swings. She had low energy levels throughout the day and faced lot of hunger pangs. Poor sleep was causing stress and leading to poor eating habits which affected her academic performance. Hence, she was looking for a program which suited her study routine, satisfied her cravings and improves her overall health and mind.

After analyzing her food diary, we saw that she was consuming lot of simple carbohydrates foods and low protein sources throughout the day. Such patterns were leading to high sugar levels throughout the day with sudden spikes in the insulin levels causing insulin resistance. Her meal timings were also off with improper portion sizes depicting poor food choices and patterns. This was leading to poor concentration and feeling lethargic at college hours and during self-study at home and increased hunger pangs later in the night causing late evening snacking which further disrupted her sleep cycle. We designed an optimum macronutrient ratio for her which suited her health goal, caused fat burning along with meeting her study and routine need. Her day started with healthy protein consumption like fruits, eggs with vegetables, a slice of cheese with carbohydrate breakfast of her choice which was also scrumptious so that she can kick start her day without feeling lethargic and fatigue. This also helped her focus on college lectures. We also provided her favorite foods in a healthier version to suffice her taste palate and mood. For example, we suggested her pasta with lot of veggies and healthy sauces. We also allowed her pizza and biryani making sure to provide with correct protein combination along with tasty salads to minimize the sugar spikes. We ensured adequate, good quality proteins for her such as eggs, chicken, healthy protein smoothies, , etc. as she is in her growing age and is a teenager. We suggested her an evening exercise routine suitable to her college timings and health requirements along with ensuring optimum protein intake post workout. Her sweet cravings were also met since we suggested healthy recipes such as almond flour cakes and pancakes, yoghurt based recipes, energy ballswhich had natural and healthy sugars. We made sure her dinner got over by 7-8pm so that a fasting period is maintained. We ensured that her body goes into fat burning mode along with meeting her macronutrient requirements as per health goal and education priorities. Healthy protein rich snacks were suggested like hummus and vegetable sticks so that she can eat it during late night studies or for evening snack. We provided her with 5-6 servings of vegetables and fruits throughout the day for PCOS management along with ensuring optimum micronutrient needs to eliminate deficiencies. Complex carbohydrates were given in order to ensure optimum fiber, vitamin needs and to manage insulin levels throughout the day. We made sure that her meals were innovative, fun, palatable and healthy. Continuous support and monitoring was done to help her at the best.



Results:

With our support and guidance, she lost 33kgs in 20months. Her cravings and hung pangs resolved. Her fasting insulin score reduced to 7. Her period cycles were regular. She felt very energetic and mood swings disappeared and her concentration improved. She did not feel stressed about her periods any more as there was no dip in energy and there was no discomfort. Her sleep quality improved and her washroom cycles got better. She is now able to sustain her lost weight and improved health even during traveling, social gatherings and with her further studies.
