Case history- Rani was a 36-year-old central government employee who had gained weight during her first pregnancy. She was keen to have another baby. At 35, she finally became pregnant after a few failed attempts; sadly, she miscarried in the sixth month of her pregnancy. Before she tried for another pregnancy, her gynaecologist recommended she reduce her weight and alter her lifestyle. She also was overweight, had a history of PCOD, and had mild gestational diabetes when she was pregnant with her second child.

We gave her an insulin-resistant wholesome diet with adequate protein to help her achieve her overall objectives. A variety of vegetables in every meal was introduced to reduce her insulin resistance. About 500 grams in a day of not so cooked vegetables in the form of salads, stir-fries, sautéed, smoothies, and soups were given to add diversity and fun to vegetable consumption. Good proteins like eggs, fish, tofu, curd, and mutton bone soup were introduced in every meal. Iron & ferritin-rich food in the form of dates, green leafy vegetables, and tofu were given to ensure that the body was prepared for a healthy pregnancy. She is an insulin-resistant body type, therefore calcium-rich foods in the form of casein protein were given. Good fats from ghee and seeds helped lower the glycaemic load. Since she is a government official and had very little time for exercise, and was preparing the body for a healthy pregnancy, we asked her to go for out-of-comfort zone walks for 45 minutes 3 to 4 times a week and yoga twice a week. She would get sugar cravings before her menstrual cycle therefore, we suggested options of healthy sugars such as dates, nuts-seeds laddoos, jaggery-based chikkis, dark chocolate, flavored yogurts, honey almonds, etc.



Results- She lost 8 kg in 4 months, her HbA1c dropped from 6.1 to 5.5, and her fasting blood sugar levels were within the 90 range. She got pregnant 4 months after she started our program. She had a normal pregnancy with no complications and no gestational diabetes, and she gave birth to a healthy baby girl.
