Case History: A 44-year-old named Kamal, who is passionate about marathons, running, and other sports, came to us with complaints of low stamina, gas, and bloating. Due to his decreased stamina, he was unable to participate fully in activities like squash, weightlifting, and 10 km runs, which he enjoys. His BMI is 20.36 kg/m², indicating he is slightly underweight for his height. Blood tests revealed stage 2 chronic kidney disease with a GFR of 85 mL/min, as well as signs of fatty liver. Additionally, he showed elevated uric acid levels and high normal urine specific gravity, likely due to impaired kidney function. His vitamin B12 levels were excessively high, approaching toxic levels. Consequently, he was seeking a diet plan to eliminate gas and bloating while boosting his stamina and energy levels, enabling him to engage vigorously in his favorite sports and activities.

As Kamal embarked on his nutritional journey, it was crucial for us to address his distinct medical conditions while creating a unified and sustainable dietary plan. This plan needed to support his energy and stamina levels. Listening to his dietary and lifestyle recall, we observed that his water intake was suboptimal and diet was a high protein diet that lacked diversity, with the same food consumed for both lunch and dinner. He was consuming approximately100 grams – 120 grams of protein every day with a body weight of 64 kgs. This needed to be replaced with an adequate and balanced diet, ensuring adequate protein split into 4 meals while ensuring that potassium, phosphorus, and sodium levels were kept within the lower limits of the Recommended Dietary Allowance (RDA). We ensured that he consumed adequate water of 3 liters throughout the day and not just during activity. We also ensured the inclusion of high-quality protein split into all 4 meals, providing no less than 0.8 grams per kilogram of body weight and later increased to 1- 1.2 grams per ideal body weight depending upon his exercise regime. To meet his daily protein requirements, personalized protein powder, hemp milk, and date seed paste were included in his diet. Animal protein sources such as chicken, fish, and eggs were also incorporated.
To improve his gut health, we added fiber to his diet through salads, vegetables, and fruits low in potassium, such as papaya, dragon fruit, muskmelon, apple, berries, peach, and pineapple. Probiotics were included in the form of hemp milk, kefir, hung curd or yogurt. To cater to his specific dietary requirements, we incorporated complex carbohydrate sources like hand-pounded rice, quinoa, and millet. Food containing gluten, and lactose were restricted to improve the symptoms along with this generous serving of cooked vegetables prepared in innovative ways to meet his fiber and micronutrient needs, supporting his overall health goals. To prevent hunger pangs in the evening, we recommended healthy snack options like dhal tikkis, pea protein shakes, a one-pot meal or tofu with sautéed vegetables, providing him with structured guidelines and meal plans for optimal food selection.
He was advised to start his day with 1-2 glasses of warm water and to stay hydrated by drinking a glass of water every hour, emphasizing water's role in boosting metabolism. For promoting health, we recommended cooking vegetables and curries in cold-pressed peanut oil, minimizing spices, avoiding deep-frying, and also maintaining 3 to 3.5 hours gap between meals, and ensuring at least 1.5 hours gap after meals and before exercise.
Results:
Kamal experienced remarkable improvements in energy and stamina after following the diet plan. This transformation allowed him to play squash three times a week, engage in weight training thrice a week, and complete a 10 km run weekly, all with optimal energy levels and swift recovery. His gut health significantly improved, eliminating bloating and post-meal gassiness, while enhancing regularity in his bathroom routines.
His kidney function also saw notable progress, as indicated by an increase in GFR from 85 mL/min/1.73m² to 99 mL/min/1.73m². This improvement underscores better waste filtration and overall kidney health. Moreover, Kamal's liver function tests and lipid profile showed positive changes. His total cholesterol dropped from 214.5 mg/dl to 186 mg/dl, and LDL levels improved from 133 mg/dl to 117 mg/dl, reducing his risk of cardiovascular issues. Notably, his uric acid levels decreased from 8.24 mg/dl to 5.9 mg/dl, signaling improved insulin sensitivity and lower risk of conditions like gout.
Additionally, his vitamin D levels surged from 21.19 ng/ml to 55.29 ng/ml, benefiting bone health and immune function. Conversely, his Vitamin B12 levels, initially high at 848 pg/ml, stabilized to a healthier 490 pg/ml, mitigating the risk of toxicity.
Overall, Kamal reported enhanced vitality and stamina, achieving a 10 km run in an impressive 53 minutes—a feat he hadn't accomplished in years. His improved gut health and kidney function, along with better liver and lipid profiles, signify comprehensive health gains and reduced risk of chronic ailments.
