Case History: Kavya and Anuj approached us seeking a comprehensive dietary plan. Kavya, aged 45, has been a professionally active individual and is a senior leader in the corporate world.She was diagnosed with polycystic ovarian syndrome (PCOS). Her BMI of 27.68 kg/m2 categorizes her as obese, and her blood profile suggests prediabetic tendencies progressing towards hidden diabetes. Additionally, she has been on hypothyroidism medications which made a weight loss journey more difficult. Her blood parameters also indicated low-normal iron levels and follicle stimulation hormone. Despite maintaining an active lifestyle, Kavya has encountered a weight plateau and was keen to lose 10 kg while effectively managing her PCOS condition.


Anuj, a 47-year-old working professional, was also keen to lose 8 kgs. His blood reports indicated low normal platelet count, elevated creatinine levels, and an increase in urine epithelial cells, deficiencies in vitamin D and B12 that needed correction. Furthermore, Anuj had elevated LDL and low HDL levels, putting him at risk for future cardiovascular disease. He also had low normal iron levels that required attention. He aimed to achieve weight loss and effectively manage his overall health.


Kavya and Anuj, as a couple, shared an aspiration to trek to Mount Everest Base Camp. Therefore, they both aimed to achieve peak physical fitness and effectively manage their health conditions to complete the trek successfully. Before embarking on this physically and emotionally demanding adventure, they sought our guidance for personalized nutrition support.

Kavya and Anuj, as a couple, were eager to embark on a nutritional journey together. It was essential for us to address their individual medical conditions while developing a unified dietary plan that would be both sustainable and manageable in the long term, without separate food lists. We also kept in mind their shared aspiration to trek to Mount Everest Base Camp. During the dietary recall assessment, we observed their conscientious approach to nutrition. However, we noted areas for improvement, such as the need for increased intake of fresh fruits, fiber, and probiotics, as well as addressing the gap between their early morning detox regime and breakfast.


To meet their specific dietary needs, we ensured they incorporated high-quality protein sources such as homemade paneer, tofu, hung curd, eggs, whole pulses, and chicken, tailored to their individual preferences. Kavya preferred eggs, while Anuj preferred a non-vegetarian diet. This approach promoted satiety and effectively supported their weight management goals. Considering Anuj's love for coffee, we strategically included coffee in his diet to bridge gaps between meals, occasionally replacing it with green tea to vary his intake, all while ensuring he maintained adequate hydration.


We introduced a variety of fresh fruits into their diet as they craved for sugars and fresh fruits taken appropriately would address their sugar cravings and give them antioxidants, flavonoids and dietary fiber. We included fresh seasonal fruits such as apples, pomegranates, kiwis, pears, berries, oranges, dragon fruit, muskmelon, and papayas. Additionally, we incorporated complex carbohydrates such as ragi roti, green roti, and millets , alongside creatively prepared cooked vegetables. These additions fulfilled their fiber and micronutrient requirements, supporting their holistic health goals effectively.


Anuj would have his lunch in his office cafeteria. We educated Anuj on starting his office meals with a sufficient portion of salad and portion controlled lean protein like chicken and fish before opting for the cereals and lentils provided in the cafeteria. Additionally, we incorporated nuts and seeds as a healthy snack into both Kavya's and Anuj's diets as a healthy source of fats as they often would get late at work. To ensure adequate hydration, we advised them to begin their day with one or two glasses of warm water and to drink a glass of water every hour thereafter. To promote their health goals, we recommended cooking vegetables and curries in cold pressed seed oil or good quality oil, using minimal spices, and avoiding deep-frying. Throughout their journey, they received continuous monitoring and guidance to support their progress.



Result

Within just 4 months of adhering to the plan, Kavya experienced significant improvements in her health parameters and blood reports, despite frequent international travel for office meetings. Her weight decreased from 81 kg to 71.5 kg, and her BMI dropped from 27.68 kg/m2 to 25.4 kg/m2 (cross check bmi once. Weight is correct). Notably, there was positive progress in her thyroid profile: T3 decreased from 1.2 to 0.98 ug/ml, T4 reduced from 11.8 to 10.2 ug/ml, and TSH decreased from 6.4 to 4.6 ug/ml. Her vitamin D levels also rose from 59.5 to 77.2 ng/ml.


Following the dietary regimen, Kavya reported improvements in her cravings and hunger patterns. Her digestive cycle, initially suboptimal when she started the program with the introduction of complex carbohydrates, showed phenomenal improvement by the end of the program. She noted increased energy levels and found herself adhering more effectively to her exercise routine, which included morning strength training, cross fit, aerobic activity and a 40-minute evening walk.


While Anuj successfully achieved his goal of reducing his weight from 88 kg to 80.5 kg. His motivation stemmed from his aspiration to trek to Mount Everest base camp.
