Case History: Kanika had lost about 10 kgs through intermittent fasting. However, over a period of time, her cravings started getting worse. Initially, she was able to sustain the weight loss with willpower. After a while, her willpower gave up, and she slowly began to yield to her cravings and started gaining weight back. Additionally, as a physical fitness teacher, she was expected to be active throughout the day. Her eating pattern was also making her feel sluggish and fatigued throughout the day, and by the end of the day, she would feel completely drained out. Her BMI was 30. When she came to us, she was quite terrified as she did not have pleasant experiences and would spend lots of money on different diet programs, but nothing yielded results. She wanted to lose weight, feel healthy and have a sustainable healthy lifestyle. She was diagnosed with significant insulin resistance and had been taking 1500 mg of metformin for 17 years to treat PCOS and get regular periods. She was unable to determine which foods would be best for her body. Her high insulin resistance and medications hindered her ability to live a healthy lifestyle. She followed a restrictive diet and did the gym, which put her body in a negative protein balance, which could further harm her bones and overall fitness.

She wanted to continue to follow the exact meal timings of her current regimen. So we followed a three-meal strategy. We gave her a low-calorie, fibre-rich, dense protein meal that was simple for a working mother in the UK. Her first two meals included 80 percent of her body’s protein requirements and a perfect ratio of macronutrients to nourish her body. The 3rd meal was a meal that had maximum carbs. That was also when she would be taking her metformin, and her body needed carbs to support her medication. She was not keen to remove metformin from her life as she had used it for the past 17 years to manage her PCOS. All the meals were given, keeping in mind her time availability and family routine. It was made sure that she does not go in negative protein energy balance along with losing weight. She was provided with healthy quick options rich in protein when there was a shortage of time. Her diet was focused on decreasing insulin resistance so that her medication was reduced, which ultimately will help treat PCOS and lose some weight. During follow-up calls, We taught her and educated her about practising mindful eating and choosing correct foods by listening to her body and hunger signs. Constant support and guidance were provided. We taught her to manage her healthy eating even while travelling and social gatherings.


Results

As a result, she lost 6.5 kgs in a span of 2 months in spite of a busy work schedule and without physical activity and with the social indulgences of Diwali, Thanksgiving, long weekends and festive weeks of Christmas and new year. As we helped her manage her overall calorie consumption in the week, balancing feasting and regular day routines, and she was also losing weight, she was in a better head space and peaceful for the first time in years. She managed to work on her health despite her mood swings, eating out, and disturbed routine. Her cravings became negligible. She started feeling light. Her hunger pangs and behaviour improved. She began to understand her body and its needs. Over the six months, her metformin was reduced to 500 mg. She learned to manage having different food over a period of time.
