Case History: Aruna, a homemaker, was suffering from grade 3 obesity with Hba1c level of 7.7%. She started gaining weight after her second pregnancy. She has been suffering from high blood pressure and sleep apnea for the past ten years. She was diagnosed with diabetes a couple of years back. Due to excess weight and knee replacement surgery, her movement became restricted, and she could barely walk to the washroom and just about manage her basic household chores. Over a period of time, she reached 115 kgs. The excess weight, along with limited movements and low energy due to medical conditions, made her more lethargic. Her doctor asked her to lose weight as her knees and legs could not support her weight. She wanted to lose weight and manage her diabetes and other health parameters to achieve sustainable health in the long run. She wanted the right motivation and guidance to change her mindset regarding eating and lifestyle habits.

Because of such a pattern, she was gaining weight, and her sugar levels were slowly and steadily increasing. By analysing her dietary recall, we saw that her diet consisted of simple carbohydrates and foods causing insulin spikes. She also consumed inadequate protein, which caused increased hunger throughout the day, leading to nibbling, snacking and fried food consumption. Her dinner used to be around 9.00 – 9.30 pm as she wanted to make sure that all family members were eating on time. As there was very minimal protein intake, consumption of empty calories throughout the day and erratic eating behaviour, her metabolism had dropped. This caused her lifestyle to be more sedentary than others. Hence, we started first by correcting her vitamin D and B12 levels so that they can aid her in better digestion and energy levels and, therefore, support weight loss. Then we provided her a plan consisting of low calorie, adequate protein and amino acid profile, complex carbohydrates in the form of vegetables and whole grains and optimum healthy fats. We created a calorie deficit to initiate the fat-burning. Before she started the program, Deepika did a coaching session on mindfulness, body types, and lifestyle habits causing weight gain. She explained to her the concept of weight gain and poor insulin resistance relating to factors such as age, metabolism, mindful eating, proper chewing of foods, hormonal changes, and sustainability. We taught her the correct lifestyle and eating behaviours. Wherever required, we gave her appropriate cooking methods to get optimum nutrition.

Meal replacement shakes were incorporated to support adequate amino acids for weight loss. Mutton bone soup, fish cutlets, etc., were suggested by Fit4Life Kitchen. Protein nachos and murkus from the Fit4Life Kitchen were given to support her cravings and nibbling. Date cubes were given to support her sugar cravings. We gave her sauté vegetables, soups, salads, and sabzi to meet her vegetable intake. Complex carbs and efficient quality protein combinations were suggested to maintain her sugar spikes. Quinoa, jowar roti, brown rice, pulao, chicken, eggs, nuts and seeds are a few foods that were incorporated. Replacements and innovative recipes were shared whenever needed. Slowly, we included her favourite foods more healthily and mindfully. Constant motivation, support, and guidance were given so that she could follow and sustain her new healthy eating pattern and lifestyle.



Results:

Within five months, she lost 13 kg. Her Hba1c levels came down to 6.4% in 3 months. Due to excellent results in weight loss, joint pains and knee pain disappeared. Her hunger levels improved, and washroom cycles got better. She now understands the importance of eating healthy and improving overall health. She recently went on an international holiday, and she was very happy that she could walk around comfortably without any discomfort. The icing on the cake was that she indulged in her favourite foods with mindfulness, and she also did not gain any weight despite her indulgences. Her other insulin resistance parameters also significantly improved.
