Case history: Veena, a homemaker, had gestational diabetes during her 2nd pregnancy. She assumed that if she lost the excess pregnancy weight, her sugar levels would come under control once again. After one year of her delivery, she tried fad diets and starvation to lose the excess weight. While she lost 8 kgs, she was traumatised to see her sugar levels were still in the prediabetes range with an HbA1c of 5.8. She would skip meals such as dinner and eat a bowl of fruit, which would make her more hungry. She would be anxious most of the time because she was not able to manage her health due to poor knowledge regarding food and nutrition. She looked forward to eating her favourite foods, feeling satiated after meals, and still reducing her HbA1c through sustainable nutrition programs and lifestyle habits even while travelling and social gatherings.

When we analysed her food diary and lifestyle habits, we saw that she was starving to lose weight and maintain her sugar levels. On many days, she would sleep hungry and be frightened to eat food as she was not sure if her sugar levels would increase. Her diet lacked variety, was mainly carbohydrate-rich, and lacked enough protein, which caused hunger pangs and insulin spikes. She had a habit of consuming a lot of tea to kill hunger, due to which she experienced sugar spikes and felt lethargic. We first started with treating her vitamin deficiencies through food and supplementation. Then, we curated a personalised meal plan which consisted of an appropriate macronutrient ratio suitable to her body type and health goal. Her meals were designed to manage her glucose spikes and provide her with satiety and pleasure. Efficient optimum protein-rich foods, such as tofu, soy, eggs, mutton, etc., were suggested. Fibre-rich foods in vegetables and complex carbs were incorporated, giving her a feeling of fullness and helping her maintain portion control. Brown rice, dosa, green rotis and vegetables in soups, salads, curries, and sautéed were suggested. Three main meals and one small snack were designed to induce correct fasting and feasting periods. Essential micronutrients were ensured by including low glycaemic rich fruits such as berries, guava, pomegranate, dragon fruit, etc. Healthy fats such as ghee superseeds were formed to meet fibre needs and balance sugar levels while inducing satiety. Customisation was done per her travel schedules, and tips were advised to ensure healthy eating and enjoying the travel. She was given real-time monitoring support and nutrition education on sustainable healthy eating and lifestyle modification to provide maintenance.


Result:

With our nutrition support and guidance, her Hba1c reduced to 5.1, and she was no longer pre-diabetic. She learnt the importance of food combinations. Her eating behaviour and lifestyle improved significantly. She further lost 3 kgs while eating more food, feeling satiated through the day, and having a sound sleep. Her intake of tea was reduced to 1-2 times a day. She enjoyed sweets and other pleasures while managing her health during festivals and social gatherings. Her energy levels, washroom cycles, water intake, and hunger levels are normal since she has gotten comfortable and healthier with the plan.
