Case history: Venkatesh, a software engineer, was suffering from several health issues for which he took various medications. He was suffering from thyroid dysfunction, for which he was taking medication. His blood reports showed high cholesterol and triglyceride levels and high hbA1c levels. He also had high creatinine levels, indicating poor kidney health. The majority of his health parameters were haywire, causing hormonal and insulin imbalances due to which he felt lethargic and weak. After analysing his dietary recall and noting his lifestyle pattern, we saw that he had an appropriate caloric intake, which helped him maintain an ideal BMI range. Still, the composition of calories from different food groups, such as protein, good fats, and complex carbohydrates, needed to be more balanced. His diet was predominantly refined carbohydrate-rich meals such as South Indian tiffin for breakfast and dinner and rice, curry, dhal and curd for lunch. His diet was deficient in complex carbohydrates, micronutrients, protein and good fats, causing imbalances in his insulin activity and contributing to insulin resistance. All these factors were causing inappropriate thyroid functioning. His gut health was also compromised due to poor eating habits. Hence, we carefully designed a customised plan for him that took care of his insulin resistance condition and hormonal functioning, as well as his health goals, lifestyle, and work routines, so that he could follow the plan with ease and comfort. We provided him with good-quality proteins such as eggs, fish and tofu to support his poor kidney profile and reduce his insulin resistance. Complex carbs such as various millet, green rotis, pound rice, and sufficient soft cooked vegetables in innovative form were incorporated to meet his dietary fibre and micronutrient requirements and support his health goals. We gave him healthy snack options such as corn chats, chole chats, and guacamole with vegetable sticks or nachos in the evening to avoid hunger pangs. We even guided him with the correct foods while he was eating out and provided him with structured guidelines and a plan for choosing foods while travelling and socialising. He likes South Indian tiffin; hence, we incorporated it with better combinations and healthier versions such as dhal dosas, quinoa chillas, and pesarattu, which have sufficient vegetables and proteins. We suggested a suitable exercise regime and asked him to incorporate it into his routine gradually. Continuous monitoring and guidance were provided.



Results:

Within three months of following the plan, his health parameters and blood reports significantly improved. His TSH levels came down to 5 from 15.4, and hbA1c levels came down to 5.7 from 6.6. His creatinine levels reduced to .9 from 1.3 and is in the normal range now. His cholesterol levels improved from 164 to 55 and TG from 211 to 96. He also experienced a significant change in his energy levels and mood. He felt more active and alive than before. He can now manage his improved health despite socialising, eating out and travelling. His doctor gradually reduced his medication and the dose.
